5 Things We Investigate After Implantation Failure (Including Chemical Pregnancies)

Experiencing a failed embryo transfer, recurrent implantation failure or a very early pregnancy loss can feel incredibly difficult, especially when it feels like you've done everything right.


Whilst implantation is a complex process and there is never one single factor that determines success, there are areas we can investigate to better understand what may be influencing your chances of a healthy pregnancy.


When working with clients who have experienced implantation failure or chemical pregnancies, these are some of the key areas we consider.


1. Starting with embryo quality


Before focusing solely on the environment, it’s important to consider the embryo itself.

Successful implantation depends on a number of factors, including embryo development and chromosomal integrity.


Egg and sperm quality can influence:

  • Embryo development

  • Chromosomal health

  • Implantation potential


This is why supporting egg and sperm health in the months before conception or treatment can be so valuable.


Areas we focus on include:

✨ Lifestyle optimisation

✨ Reducing oxidative stress


If you have already transferred a PGT-A tested embryo, this can provide additional information about chromosomal status. In this case, we would often move towards investigating other factors, including the uterine environment.


2. The reproductive microbiome


The role of the reproductive microbiome is an area of growing interest within fertility research.


A healthy vaginal and uterine microbiome is typically dominated by beneficial Lactobacillus species, which are associated with:

  • Improved implantation outcomes

  • Better pregnancy outcomes

  • A lower risk of miscarriage


When there is an imbalance (known as dysbiosis), this may influence:

  • Inflammation

  • Immune signalling

  • Endometrial receptivity

  • The environment needed for implantation


This is why reproductive microbiome testing may be considered for some individuals, particularly where there has been recurrent implantation failure or pregnancy loss.


3. Progesterone and hormonal balance


Progesterone plays a vital role in preparing the uterine lining for implantation and supporting early pregnancy.


However, it’s important to remember that progesterone is not just about one number, timing, individual circumstances and your treatment protocol all matter.


Suboptimal progesterone levels may contribute to difficulties with implantation or early pregnancy support.


Alongside progesterone, we also consider the wider hormonal picture, including:

  • Ovulation

  • Oestrogen balance

  • Thyroid health

  • Blood sugar regulation

  • Nutrigenetics where appropriate


Hormones work together as a team, which is why looking at one marker in isolation may not provide the full picture.


4. Does your body have everything it needs?


Early embryo development is an incredibly energy-demanding process, and the body requires a wide range of nutrients to support optimal cellular function.


When assessing fertility nutrition, we may look at:

🩸 Iron and ferritin levels

🧬 Folate, B12 and other B vitamins

⚡ Methylation pathways

🔥 Inflammation

🩺 Metabolic health

🍽️ Overall nutritional status


Certain nutrients are involved in processes such as DNA synthesis, energy production and antioxidant defence.


However, supplements are only one part of the picture.

Nutrition, digestion, sleep, movement, stress management and overall health all influence how well the body can support implantation and early pregnancy.


5. Immune and endometrial factors


Implantation requires a carefully balanced immune response.

The immune system needs to support implantation while also allowing the embryo to develop, making this a highly regulated process.


For some women experiencing recurrent implantation failure or recurrent chemical pregnancies, further investigations may include:

  • Cytokine balance

  • Natural killer (NK) cell activity

  • Endometrial inflammation

  • Decidualisation markers

  • Endometrial receptivity testing


These factors may contribute to an inflammatory environment that makes implantation more challenging for some individuals.


However, investigations should always be guided by your personal history and carried out alongside an experienced fertility specialist where appropriate.

A healthy, well-vascularised endometrium helps deliver oxygen and nutrients needed during implantation.


Factors that can influence blood flow and vascular health include:

  • Regular physical activity

  • Nitric oxide production

  • Cardiovascular health

  • Inflammation levels

  • Genetics

  • Smoking

  • Certain medical conditions


Supporting overall vascular health is another important piece of the fertility puzzle.


Implantation isn’t just down to luck
